Information this website collects

We only collect information when you choose to send it to us through one of our two forms:

Contact form: your first and last name, email address, phone number (optional), a subject and your message. If you check “Sign up for news and updates,” we also note that you would like to hear from us.

Opening updates signup: your name and email address.

Our form tool also records a few technical details with each submission, like the date and time, your IP address and your browser type. This helps us spot and block spam.

We do not ask for, and do not want, details about your symptoms or health history through this website. Any clinical or health information is exchanged through the Tebra patient portal or directly with our clinical staff, not through this site.

How we use it

We use what you send us to reply to your message, schedule an appointment or answer your question. If you signed up for news and updates or opening updates, we will also send you occasional clinic news, like our opening date and open house. You can stop those emails anytime.

We do not sell or rent your information to anyone.

How we store it

Form submissions are emailed to our clinic inbox and saved in our website’s admin area, which only our staff can access. We keep them as long as we need them to follow up with you or send the updates you asked for. You can ask us to delete them at any time.

Protected health information

Wave Medical Center is a HIPAA-covered healthcare provider. Health information you share with us as a patient, whether by phone, in person, or through the Tebra patient portal, is handled under our HIPAA obligations and our separate Notice of Privacy Practices, available at the clinic. This website’s general contact form is not a secure channel for health information; please do not include symptoms, diagnoses, or medical history in a website message. Call us instead for anything health-related.

Cookies and tracking

This website does not use advertising or analytics tracking. Our website host and form tool may use basic cookies that the site and forms need to work.

Third-party services

Scheduling and the patient portal are provided by Tebra, a third-party vendor. Tebra’s own privacy policy governs information you submit directly through the patient portal.

Our website is hosted by GoDaddy, and our email runs on Microsoft 365. These providers store information on our behalf so we can run the site and our inbox.
